ON CARNIVAL DAY THERE WILL BE STRICT ENFORCEMENT OF ATTIRE.

• –IF YOU REGISTERED FOR A COSTUME FROM MADD COLORS YOU MUST BE DRESSED IN A MADD COLORS 2017 COSTUME.
• –IF YOU REGISTERED FOR THE T SHIRT SECTION YOU MUST BE ATTIRED IN THE MADD COLORS 2017 T SHIRT.

A NON-REFUNDABLE deposit of $50 is required for pre registration. This deposit is necessary to secure your costume and your participation with Madd Colors Carnival for the Atlanta/Dekalb Carnival 2019.

Kindly note that once you have selected your designated costume, there will be no switching.  I understand that I am responsible for the payment of my select the costumes as laid out in this registration form and I am fully understanding of the terms and conditions. I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN ANY/ALL ACTIVITIES ASSOCIATED WITH Madd Colors Carnival for the Atlanta/Dekalb Carnival 2019, and all who are being represented, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.

I certify that I am physically fit, have sufficiently prepared or trained for participation in this activity, and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in this activity. I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity in which I may participate, and that it will govern my actions and responsibilities at said activity.

I, the undersigned do herby consent and agree that Madd Colors Carnival, its employees or agents, have the right to take photographs, videotape, or digital recordings of me and to use these in any and all media now or hereafter known. I release all rights to exhibit this work in print and electronic form publicly or privately and to market. I understand that there will be no financial or other remuneration for recording me. I represent that I am at least 18 years of age. If under the age of 18 years old you must bring a parent or guardian to sign a release form, before the costume is released. Registration is only considered complete after full payment.

Madd Colors Carnival costumes are custom-made items, therefore please be advised that once payment is made for pre registration, or registration in general, there will be

NO refunds issued.
NO exchange or switching of sections.
NO changing of measurement or sizes

By purchasing or attempting to purchase any products and/or services offered or advertised by Madd Colors Carnival, you are stating that you fully and unconditionally agree to all policies, disclaimers, terms and conditions detailed herein.
